Please read this statement carefully Due to Privacy Legislation, we require your consent to collect personal information. This practice collects your information in order to identify your medical record and provide an accurate, quality health service. This means that we will use the information you provide in the following ways:
- Administrative purposes in running a specialist medical practice: including calls using phone numbers and names you provide us. The best therapy is based on sound communication with all involved in your child’s care including doctors, therapist, teachers and family.
- Billing purposes, including compliance with NDIS, Medicare and Health Insurance Commission requirements.
- Disclosure to others involved in your medical care, including treating doctors, specialists, therapists and other team members. This may occur through referral to other services, in reports and during case conferences, and staff training.
- Under mandatory reporting laws and other legislation it may be necessary to provide details to child protection or police services. Whenever possible these matters will be discussed with parents first.
